- Assist with the admission, transfer, and discharge of patients
- Document patient information and maintain accurate medical records
- Educate patients and their families on healthcare procedures and self-care techniques
- Monitor and report any changes in patient condition to the appropriate healthcare professionals
- Ensure compliance with all applicable laws, regulations, and policies
Skills:
- Proficient in medication administration and dosage calculations
- Experience working in a nursing home or long-term care facility
- Strong case management skills to coordinate patient care across multiple providers and settings
