Staff Nurse - RN - Home Care Admissions jobs in North Carolina
Staff Nurse - RN - Home Care Admissions evaluates patients referred to home care services and determines the types of services to be provided. Interviews the patient and family to determine appropriateness for services. Being a Staff Nurse - RN - Home Care Admissions obtains initial and ongoing referral documentation from physicians, insurers, and providers. Coordinates the development of patient home care plans that specify the frequency and duration of the services provided. Additionally, Staff Nurse - RN - Home Care Admissions initiates care with service providers. Completes admission, referral, and insurance forms. Utilizes health record management software. Typically requires a bachelor's degree in nursing or equivalent. Requires an RN license. Typically reports to a manager or head of a unit/department. Staff Nurse - RN - Home Care Admissions's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. - Follow a designated care plan in accordance with patient's needs
- Make home visits to clients in designated geographic territories
- Perform assigned duties, including administration of medication, wound care, treatments, and procedures
- Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
- Follow up with, execute, and properly document doctors' orders
- Perform client assessments as necessary
- Case management and coordination
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet
- A current license as a Registered Nurse in North Carolina.
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
