May require a bachelor's degree. Additionally, Regional Support Manager typically reports to top management. The Regional Support Manager typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget. Capable of resolving escalated issues arising from operations and requiring coordination with other departments. To be a Regional Support Manager typically requires 3+ years of managerial experience. HealthCare Support is actively seeking a Field RN Case Manager to fill an opening in Providence, RI.
Daily Responsibilities for a Field RN Case Manager:- Communicate and collaborate with the interdisciplinary care team
- Assist the provider/team with various care coordination activities
- Assist the team with benefit determination associated with Medicare part A, B, and D benefits
- Responsible for collaborating with the primary advanced practice clinician, (APC) and facility to identify and respond to patient change in condition
- Schedule and participate in family conferences, team meetings, and team case presentations.
- Coordination of training, tracking, and compliance with quality measures
- Assist in creating and conducting in-services for selected audiences
- Assist Providers in assuring complete and accurate documentation and coding, medication reconciliation, nursing rounds, assessment, and patient documentation
- Assist in managing members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, interventions
- Assist in managing members during a short term sub-acute rehabilitation episode
- Shift: Monday to Friday, 8 - 5
- Active and unrestricted license in RI
- Minimum 1 year working in a hospital or Long-Term Care facility as an RN
- Associate degree or higher in Nursing (RN)
- Certified in Basic Life Support
- Ability to complete physical requirements, including lifting a 30-pound bag and ability to perform physical assessment
- Access to reliable transportation and ability to travel between facilities
- Full COVID-19 vaccination is an essential requirement of this role. UnitedHealth Group will adhere to all federal, state, and local regulations as well as all client requirements and will obtain necessary proof of vaccination prior to employment
- 1 year of clinical experience in practice (long-term care setting preferred)
- Understanding of Geriatrics and Chronic Illness (preferred)

Pay Details: $47.00 - $56.00 an hour
