    Job Description

    coordinating care, managing medications, physicians appointments, crisis intervention with hospitalizations, community involvement to explain who we are and what we do, must have strong collaborative experience and the ability to challenge the system when they don't do what they should do  eventually will be certified as a dementia practitioner and crisis interventionist paid for by the company. This is a very independent role for the right person.

    Salary commensurate to experience Flexible hours
