HOME CARE POSITION in Jersey City, Hudson County.

Outreach PT, OT and SLP is one of the leaders of in home out patient physical, occupational and speech services in New York and New Jersey. Due to our continued growth , Outreach is looking to bring on additional motivated and passionate Therapists to work with the adult and geriatric populations. • We aim to bridge the gap between a home health agency and an out patient clinic.
• We provide all treatment in patients’ homes, Senior Residences Assisted Living Facilities, or office setting

Responsibilities of Physical Therapist:

• We assess patients home environments for safety and environmental barriers
• provide fall risk assessments
• address physical underlying impairments and how it affects overall function
• ambulatory aid and adaptive equipment training
• education to patient and caregiver regarding compensatory strategies and or a Home exercise program

Qualifications of Physical Therapist:

• In Home - Masters degree a minimum requirement
• 1 year of experience working with older adults preferred.
• Home care experience a plus.

Job Type: Per Diem or Part-time position available

Pay: $60.00 - $74.00 per hour
