This position is a full time (0.8 - 1.0 status), day shift RN Nurse Navigator position in the Medical Oncology Department.
POSITION SUMMARY
The focus of the RN Nurse Navigator position is to plan, coordinate, and provide specialized wound, ostomy, and continence care along with wound care team including education for patients and families. Standardize care for patients with critical limb ischemia. Perform risk assessments, facilitates appropriate testing ordered by license providers, and supports, coordinates, and educates patients and their families as it relates to disease, procedures and testing. Serve as resource person on nursing practice in skin, wound, ostomy, and continence care for hospital staff and community. Coordinate, implement, and maintain a comprehensive pressure sore prevention program in hospital. Coordinate comprehensive ongoing education and staff development in all aspects of skin, wound, ostomy and continence care. Help coordinate with measuring outcomes and quality improvement. Plan, coordinate, and provider Anorectal Manometry testing as well as collaborate with out-patient wound care team.
