Minimum Data Set (MDS) Coordinator (LPN) assesses long-term care patient health needs in collaboration with the Minimum Data Set (MDS) Coordinator (RN). Assists in developing a comprehensive care plan. Being a Minimum Data Set (MDS) Coordinator (LPN) maintains medical records and ensures documentation is present for coding. Adheres to federal, state, and local regulations and organizational policies and procedures. Additionally, Minimum Data Set (MDS) Coordinator (LPN) requires a high school diploma. Requires Cardiopulmonary Resuscitation (CPR) and Licensed Practical Nurse (LPN) certification. Typically reports to a mds rn. Responsible for the development and the timely completion of the MDS, Raps, Triggers, and care plans. Ensure that the data is appropriate to the needs of the resident based upon the assessment and is in compliance with State and Federal regulations.
Degree in nursing, social work or dietetics required. Must have a minimum of two years of clinical experience, preferably in a Long Term Care setting. Must be knowledgeable of the MDS process.
