    Medical Record Specialist

    Full-Time, Monday-Friday, 8AM to 5PM, No Weekends! Major Holidays off and paid!

    Job Title: Medical Records Specialist

    Reports To: Health Information Systems Manager

    Area of Responsibility: Process requests for release of information from the medical chart in accordance with federal and state law and assists with administrative tasks in the clinic.

    Major Duties

    • Play an active role in patient care using the provider led, team-based Patient-Centered Medical Home Care Model.
    • Ensure timely processing of medical record requests from patients of CAPWN Health Center to other medical facilities.
    • Ensure timely processing of requests for medical records from medical facilities, law enforcement, disability boards, attorneys, insurance companies, and Workers’ Compensation carriers, following privacy and legal requirements.
    • Maintain the integrity and confidentiality of all Protected Health Information (PHI) and Electronic Protected Health Information (EPHI) documented in the CAPWN medical record and mandated by HIPAA standards.
    • Assist with the purging of medical records following established policy and procedure.
    • Scan and attach inbound records, identifying information that needs to be preloaded in patient’s medical record.
    • Billing of patients and 3 rd parties for copies of medical records.
    • Review patient records with patients upon their request.
    • Participate in HIPAA Committee.
    • Establish and maintain relationships with identified service providers.
    • Maintain a positive and professional attitude, striving to openly communicate with co-workers and patients, breaking down communication barriers when necessary.
    • Attend staff training sessions and/or meetings as required.

    Minimum Qualifications

    Grade 12:

    • Experience in a health information/medical record setting p referred.
    • Strong interpersonal skills required in oral and written communication, time management, and critical thinking.
    • High school diploma or GED.
    • Above average customer service skills.
    • Ability to work independently and unsupervised.
    • Have a working knowledge of HIPAA Privacy Regulations, maintaining confidentiality in the provision of all services including written and/or oral reports.
    • Working knowledge of Microsoft Office Suite, including Word, Excel, Outlook, and Publisher.

    Grade 13

    • Must meet minimum qualifications above;
    • Two year's experience in a clinical/hospital health information setting processing requests of information (ROI) for medical records OR
    • Successful completion of Medical Terminology course or equivalent OR
    • Certified Registered Health Information Technician (RHIT)

    Grade 14;

    • Must meet minimum qualifications above;
    • Associate degree in health information management (Technology) OR
    • Five years of experience in health care setting in Health Information/ Medical Records
