Licensed Practical Nurse - Home Care administers nursing care in a home care environment under the supervision of a registered nurse or other medical supervisor. Provides basic medical care, including changing bandages, administering medication, and collecting specimens. Being a Licensed Practical Nurse - Home Care ensures the health, comfort and safety of patients by assisting with bathing, feeding, and dressing. Monitors and reports changes in patient's condition to supervisor. Additionally, Licensed Practical Nurse - Home Care requires a high school diploma. Typically reports to a registered nurse or supervisor. Requires graduation from approved LPN educational program. Requires a state license to practice. Licensed Practical Nurse - Home Care's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. Change the lives of patients/residents and their families as a Licensed Practical Nurse -- LPN. You will be part of an interdisciplinary team that includes physicians, nurse practitioners, registered nurses, wound care nurses, social workers, dieticians, speech-language pathologists, physical therapists, occupational therapists, and other to achieve the best patient outcomes and quality of life.
Position Highlights- Deliver efficient and effective nursing care while achieving positive clinical outcomes and patient/family satisfaction.
- Evaluate patients to collect data, observe condition and report changes in condition.
- Contribute to nursing assessments and care planning.
- Administer medications and perform treatments per physician orders.
- Supervise and monitor patient care provided by unlicensed staff.

- Must be a graduate of an approved school for practical or vocational ursing.* Must be currently licensed by the State Board of Nursing or pending icensed practical nurse licensure.* Experience is preferred in rehabilitative or geriatric nursing.* Must maintain current BLS/CPR certification if employed at a skilled nursing center or as required by the state of practice if an assisted living facility.
