Licensed Practical Nurse - Home Care administers nursing care in a home care environment under the supervision of a registered nurse or other medical supervisor. Provides basic medical care, including changing bandages, administering medication, and collecting specimens. Being a Licensed Practical Nurse - Home Care ensures the health, comfort and safety of patients by assisting with bathing, feeding, and dressing. Monitors and reports changes in patient's condition to supervisor. Additionally, Licensed Practical Nurse - Home Care requires a high school diploma. Typically reports to a registered nurse or supervisor. Requires graduation from approved LPN educational program. Requires a state license to practice. JOB DESCRIPTION SUMMARY The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of a Registered Nurse.
Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES 1. Provides direct patient care as defined in the state Nurse Practice Act.- Implements plan of care initiated by the Registered Nurse.
- Provides accurate and timely documentation consistent with the plan of care.
- Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
- Participates in coordination of Home Health Services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
- Uses equipment and supplies effectively and efficiently.
- Participates in personal and professional growth and development.
- Performs other duties as assigned by the Registered Nurse.
- Has 3 years nursing experience. Community/Home Health experience is preferred.
- Holds Active License as a Licensed Practical Nurse in the State of CT.
- Complies with accepted professional standards and practice.
- Demonstrates good verbal and written communication, and organization skills.
- Must have reliable Transporation, Active Driver's License and Insurance
