    Description

    The MeckHome Housing Program Case Manager provides a variety of office and field activities to manage and monitor a rapid re-housing program for young adults. Performs direct client services and compiles with related documentation and implements the RENEW Model with housing clients.

    Work involves providing supportive services to promote participants self-sufficiency, integration into the community, and permanency in housing, performing administrative tasks involved in the review and maintenance of a caseload of program participants. The principal duties are performed both in a general office environment and in the field and community where program participants reside.

    ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

    • Assess housing barriers and strengths of targeted populations experiencing homelessness to determine housing and service needs.
    • Provide mediation and advocacy with landlords on the client’s behalf to develop a workable plan to obtain and or maintain housing. Serve as an ongoing liaison between property managers and participants as well as between participants and neighbors.
    • Assist participants in locating and securing housing of their choice.
    • Provide housing case management and direct client services to a caseload of 10 to 15 households.
    • Provide housing retention services to housing clients.
    • Provide information and referral assistance regarding available support from appropriate social service agencies and/or community programs.
    • Assist in development of and encourage adherence to a personal budget through pro-active housing and budget counseling sessions; provide budget counseling and education to assist clients in establishing payments plans for bills and past debts and to assist clients in obtaining and maintaining their housing.
    • Knowledge or understanding of tenant’s rights and responsibilities as well as “strengths based” case management.

    EDUCATION And/or EXPERIENCE

    Bachelor’s degree in a related human service field required. 2 years’ experience in human service field, working with transition age youth is preferred.

    Must own a car and have andvalid NC or SC Driver's license.
