Disease Management Case Manager coordinates the overall interdisciplinary plan of care for a patient in a disease management program, from admission to discharge. Acts as a liaison between patient/family, employer and healthcare personnel to ensure necessary care is provided promptly and effectively. Being a Disease Management Case Manager responsibilities include but are not limited to documenting case progress, identifying health risks, and reporting the findings of the case study at appropriate intervals. Requires an associate's degree/bachelor's degree, and is licensed to practice nursing. Additionally, Disease Management Case Manager typically reports to a supervisor or manager. Case Manager RN - PAMC Case Management

    Under general supervision this position uses Case Management Guiding Principles in partnering with patients and physicians to assure access, services, and transition for the continuum of care. Works as team member with health care professionals, community resources, liaisons, and families to provide coordinated admission and discharge planning. Advocates and supports the case management process using the Physician-Centric Care Management Model as a guideline. Partner in identifying and achieving timely patient care needs with implementation of the plan of care that is appropriate, resourceful, and valuable. Effectively utilizes tools that measure, monitor, and manage quality and stewardship of patient care resources.

    As applicable (i.e. Acute Rehabilitation Unit assignment) coordinate rehabilitation patient services from the time of screening for admission to the Acute Rehabilitation Unit to the point of discharge to the community. Assist with collection of FIM data from team and enter into appropriate computer systems to ensure payment.

    Continuum of Care: Matching an individual's ongoing needs with the appropriate level and type of medical, psychological, health, or social care/services within an organization or across multiple organizations.

    We are hiring for the following positions in Case Management department at Providence Alaska Medical Center in Anchorage, AK.

    • Case Manager RN - Full time, 1.0 FTE, Day Shift, 40 hours per week (eligible for $5,000 Hiring Bonus for external candidates who meet conditions for payment, and Relocation assistance for eligible hires)
    • Case Manager RN - Part time, 0.6 FTE, Day Shift, 24 hours per week (no bonus or relocation)
    • Case Manager RN - Registry position, 0.01 FTE, Day Shift, hours per week are as needed (no bonus or relocation)

    Required Qualifications:

    • Nursing degree/diploma
    • Alaska Registered Nurse License upon hire
    • 2 years Clinical nursing experience

    Preferred Qualifications:

    • Bachelor's Degree in Nursing or
    • All Registered Nurses without a minimum of a Bachelor’s degree in Nursing must obtain a Bachelor’s degree or higher within 3 years of hire. Any previously agreed upon effective date for obtaining a Bachelor’s degree in nursing between individual and facility will remain in effect. Any collective bargaining contract with specific time frames for obtaining a Bachelor’s degree in nursing will supersede the above point and will remain in effect. Exceptions will be considered on a case-by-case basis at each facility by the most senior administrative nursing leader in consultation with the Human Resources leader. RNs employed prior to 11/1/17 are encouraged to obtain a Bachelor’s degree or higher degree in Nursing, but obtaining the degree will not be a condition of employment
    • 3 years Clinical nursing experience
    • 1 year utilization Management-review and Discharge-planning experience
