NTT DATA Services currently seeks a Care Management Associate-Discharge Planner to join our team for a work from home opportunity in Phoenix, AZ (must reside in Maricopa county).
Will work directly to ensure quality of work delivered, performance/productivity benchmarks are met, and all compliance related issues are properly addressed, trained and coached on a consistent basis. Speak to member regarding specialty appointments for a new member in the first 30 days. The initial 30 days are devoted to a be able to connect with a care giver, explaining the importance of specialty appointment, assisting with helping that care giver find a provider and following up to ensure and track the appointment occurred. These specialty appointments that need to be scheduled and tracked are Medical, Dental and Behavioral Health.
Role Responsibilities
-Process intake of services via email
-Researches member eligibility information, researches PCP and updates system
-Requests additional records for case managers if needed
-Protects the confidentiality of member information and adheres to company policies
-Effective communication, telephonic and organizational skills
Required Skills/Experience
*2 years customer experience or call center experience in healthcare
*1 years experience working in a Managed Care
1 years working with multiple computer systems with a strong computer literacy and the ability to navigate through internal and external systems
1 years medical terminology and concepts of care management
Preferences
-Effective communication, telephonic and organizational skills
-Must possess strong customer service skills to coordinate services
*Proficient in key boarding, typing 45 wpm with min score of 6500 alphanumeric skill set
Education: verifiable high school diploma or GED
