Decision Support Project Specialist provides technical guidance and solutions to user groups working with decision support software and data tools. Analyzes user requirements and evaluates functionality and configuration options available for existing and new software. Being a Decision Support Project Specialist delivers user training and coaching to facilitate problem solving and decision making using hardware, software, datamarts, and data warehouses. Plans, coordinates and monitors projects to prepare, test, and deploy decision support tools, reports, dashboards and models to users. Additionally, Decision Support Project Specialist troubleshoots and resolves problems. May have specialized knowledge of a particular software product tool or module that is utilized. Requires a bachelor's degree. Typically reports to a manager or head of a unit/department. Decision Support Project Specialist is a specialist on complex technical and business matters.     Prism Health is the only nonprofit LGBTQIA health center providing comprehensive primary care, behavioral healthcare, and pharmacy services in the Pacific Northwest (Oregon, Washington, and Idaho). Since opening its doors in 2017, Prism Health has become a model for culturally specific care and now serves over 1,500 patients with a wide range of services, from day-to-day primary care and pharmacy needs to targeted services like gender-affirming care and specialized population health screenings.     Prism a program apart of Cascade AIDS Project is excited to announce that we are hiring skilled and team-oriented Peer Support Specialist – Trans /Gender Diverse Services. The Prism Health Peer Support Specialist – Trans /Gender Diverse Services provides a variety of recovery-oriented services to transgender identified individuals enrolled in Primary Care or behavioral health services who have self-identified as desiring peer support. The person in this position will be utilized between Prism Health and Providence Health & Services. This position includes travel throughout the service area for meetings and client home-visits.

     

    This is a Full-Time role working at our Prism Morris location in NE Portland. The schedule for this role is Monday-Friday 8:30am-5:00pm. Must be available to work evenings and weekends for required agency events.

    Starting pay is $22.04 which is equivalent to $45,843.The Peer Support Specialist-Trans /Gender Diverse Services’ pay increases with tenure and the top of the pay scale is $25.79. This is a union represented position, so the compensation, benefits and conditions of work are collectively bargained.

     

     

    Who You Are

    • Successful completion of an Oregon Health Authority (OHA) Addictions & Mental Health (AMH) -approved Peer Support Specialist Training and certification as a Peer Support Specialist. 
    • Lived experience as a person who identifies as transgender and/or gender diverse. 
    • Knowledgeable about the lived experience of experiencing mental health and/or substance use issues and willingness to mindfully share those experiences.  
    • Successful experience working with ethnic, racial, economic, and sexually diverse populations and persons who have experienced homelessness, persons with a mental illness and/or substance abuse. 
    • Desire and ability to engage in equity and inclusion initiatives and discussions in the workplace, including but not limited to engaging in training workshops, team and department meetings, and critical discussions for how CAP can better support the communities we serve. 
    • Basic computer and keyboard proficiency using Microsoft Office software (Word, Excel, Outlook) and working knowledge of the internet. 

     

    What You’ll Do

    • Work in collaboration with Prism Behavioral Health team, Providence Health & Services, medical providers, service navigators, or other providers involved in client care. 
    • Uses a Person-Centered approach to create and work on treatment goals tailored to the specific needs of the individual receiving services. 
    • Uses own experiences as guided by training and models the principles of recovery for those they work with, including gaining greater independence, emphasize wellness and self-determination. 
    • Accompany clients to appointments to facilitate access to medical care, substance use treatment, mental health services, housing, and other services. 
    • Assists the persons they work with to identify and utilize natural community supports to decrease dependence on the traditional mental health system. 
    • Provide support to clients who are having symptoms that could result in hospitalization. Duties could include grocery shopping, meal preparation or just spending time with clients until they feel secure enough to be on their own.
