Being a Community Health Director identifies community health program related grant funding and develops grant proposals. Manages specific community health programs from planning and design through implementation and evaluation. Additionally, Community Health Director requires a master's degree in a related area. Typically reports to a top management. The Community Health Director manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure. To be a Community Health Director typically requires 5+ years of managerial experience. Deep knowledge of the managed sub-function and solid knowledge of the overall departmental function. Job Summary: Under the Manager of Medical Home, CHWs will work with the high-utilizer (frequent ER visits and/or multiple hospitalizations) subset of Primary Care Health Home (PCHH) participants, Chronic Care Management (CCM) or any at-risk OCH patient with a goal of stabilizing outpatient status and management and reducing avoidable hospital and emergency department use. CHW will help to bridge patients to resources to reduce social determinants of health that impact patients’ health and well-being. Provides informal counseling and social support, culturally appropriate health education, and basic screening services. Ability to perform duties consistent with Standards of Practice, goals and philosophy of Ozarks Community Hospital and the Employee Code of Conduct.
