    POSITION SUMMARY: This position collaborates with the other professional disciplines in the planning and providing various therapeutic interventions based on individual needs and problems. Participates in discharge planning as well as providing for other social service/case management needs. Provides individual, family, marital, group, and milieu counseling. Demonstrates performance consistent with the mission, philosophy, and goals of the unit and organization; remains flexible to changing systems; demonstrates quality and effectiveness in work habits and clinical practice; and treats staff, physicians, patients and families with consideration and respect. MINIMUM QUALIFICATIONS: EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Master’s degree in social work, psychology, or related field. 2. Current Crisis Prevention Intervention (CPI) Certification OR Obtained through next available class. PREFERRED QUALIFICATIONS: EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Current West Virginia, or applicable state, licensure as a Professional Counselor or Social Worker. EXPERIENCE: 1. Two (2) years of Social Work or Psychology Experience in a mental health setting. CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. Promotes service excellence and demonstrates commitment to improving the quality and effectiveness of the treatment process and behavioral health services, groups, activities, and individual therapies. 2. Contributes to team output, team goals and team morale. 3. As relevant to care, treatment, and services, gathers information during the bio-psychosocial assessment. Ensures bio-psychosocial data base is thorough, timely and reflects an accurate evaluation of the necessary patient data per the standards. Assists with developing the plan of care by collaborating with the patient and other members of the health care team. Identifies and addresses the patient's individual strengths and weaknesses. Plans, initiates and evaluates discharge planning or other social service needs. 4. Establishes a daily / weekly schedule of therapeutic groups and activities after collaborating with the patient(s), treatment team and physician. Communicates the weekly schedule to the patient(s). Plans, provides and documents groups and activities at least twice daily to meet the needs of patients with varying diagnoses, age groups and problems. 5. Communicates effectively with the patient, negotiating with them to focus on the goals of treatment, to overcome barriers to recovery, and work towards optimal health, self-care and functioning. Involves the patient and family as appropriate in developing / revising the plan of care to achieve positive outcomes. Ensures documentation reflects appropriate care, treatment and services carried out by the incumbent. 6. Evaluates changes in the patient's emotional, behavioral and cognitive status as compared to admission data; collaborates effectively with the team and enters the collective re-evaluation under "collaborative note". 7. Provides appropriate individual and family counseling based on the patient's individual assessed needs / problems, promoting healthy interpersonal relationships, problem resolution and adaptive functioning through use of support systems and other services. 8. Reviews the plan of care with assigned patients at least every three days. Revises and updates problems, therapeutic interventions and short term goals as the patient's condition changes. 9. As requested, performs a clinical review of the patient's medical record. Pre-certifies all admissions with appropriate payers. Transmits timely and thorough patient data to justify patient's continued stay. Documents results of clinical review interaction with payer and communicates with the business office. 10. Provides information / patient teaching related to Advanced Directives and assists the patient per policy and WV statutes to implement advance directive, living will, or other document. 11. Evaluates patients for imminent risk of self-harm or harm to others. Implements measures to contract for safety, communicating effectively with the patient, family and significant others to minimize imminent risks. Carries out the duty to warn as necessary. Assists in evaluating the need for involuntary hospitalization, serving as a mental hygiene petitioner, or witness to patient behavior.
