    Home & Health Care Management is the oldest home health care agency in Northern California. Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients.

     

    The Role of the Social Worker

    This position is responsible for overseeing the comprehensive care management activities of clients for contracts that the agency has with the Medi-Cal Home and Community Based Alternatives (HCBA) Waiver Program and AIDS Waiver Program (MCWP). Our agency manages providers and clients in an 8-county catchment area for the California Department of Health Care Services. Our program is growing and we need additional staff to assist the agency in growing and promoting quality patient care.

     

    Who You Are:

    You have a Bachelor’s degree in Social Work or an equivalent and are passionate about making a difference in the lives of clients with chronic health conditions such as HIV/Diabetes/Heart Failure.

     

    You are detail oriented, computer literate, and want to work as part of a management team supervising the care of clients who are receiving home and community-based services.

     

    You are knowledgeable about community resources and helping clients solve social determinants of health care problems and want to be valued for your social workers skills.

     

    Responsibilities:

    • As part of an Interdisciplinary team with a Registered Nurse, review Medical Records including creating a client’s Plan of treatment, menu of health services, determine if the services provided are adequate and in sufficient quantity to maintain the client safely at home with supportive services. Research community resources and facilitate referrals for these resources.
    • Make face-to-face visits or monthly telephone calls with clients to ensure that the client’s psychosocial well-being and other needs are being met. Provide home safety evaluations.
    • Document all interactions with client, team members and other health care partners in the state required software MedCompass and in Agency Matrix software.
    • Review client’s MediCal status and assist clients as needed to apply for and remain in good standing with MediCal and/or Medicare
    • Communicate with providers/ patient participants or health care department representatives both verbally and in written format regarding clients’ care needs. Utilize electronic health record (MedCompass and or Brightree/Matrix) as required by departmental procedures.
    • Institute tracking of quality indicators and reports to improve client outcomes.
    • Assist client in recruiting either IHSS workers or Waiver Personal Care Service workers, independent nurse providers, or home health agencies and the processes for on-boarding those individuals or agencies into the state WPCS or Public Authority or MediCal enrollment processes. Assist clients on finding housing, arrange transportation, resources for those that are food insecure.
    • Coordinate with Care Management team members (RN and MSW) and HCBA/ AIDS waiver clerical support staff via zoom or in person meetings as needed to review quality indicator findings and or improvement activities.
    • Attend all required staff meetings, educational trainings and communicate schedule to Management team.
    • Prepare & submit daily time cards. Document daily calls that are made to clients, their caregivers or medical providers.

     

    Essential Requirements:

    • Bachelor’s degree in social work, gerontology, health science or related degree
    • Excellent communication skills – both verbal and written
    • Ability to work independently and as a members of a team
    • Excellent organizational skills, ability to prioritize work and meet work deadlines
    • Valid California Driver’s License
    • Eligible to be insured under our liability policy with a clean DMV report
    • Your own vehicle and proof of current auto insurance
    • Computer literate and knowledge of Microsoft products
    • Professional behavior when on duty

     

    Preferred Skills, Knowledge and Certifications

    • At least one year of work experience as a social worker
    • Inquisitive nature and analytical skills
    • Ability to work with others, at all levels within the organization, and collaborate effectively.
    • Excellent problem solver
    • Fluency in Spanish

     

    Service Region/ Location:

    The role serves clients in Butte, Glen, Yuba, Sutter, Colusa, Tehama, Shasta and Trinity counties.

    This is not a remote role and requires you to work in our Chico office.

     

    Schedule:

    This role is full-time, 40 hours per week, from 8am to 5pm Monday through Friday with an hour for lunch from 12pm-1pm. No weekend work is required.

     

    Travel:

    Home health care requires you to drive to patient’s homes. You will need to be in the field approximately 50% of your time, and in the office for the other 50% of your time, depending on caseload needs.

     

    Physical Requirements:

    Conditional upon job offer is successfully passing a medical exam that tests amongst other things the ability to move, visual acuity, and hearing acuity. Physical requirement for this role specifically include

    • Sits, stands, bends, lifts, walks, and moves intermittently during working hours
    • Able to lift to 35 pounds without assistance
    • Able to drive intermittently

     

    Compensation:

    Pay range is $22 to $30 per hour. Exact compensation may vary based on skills, experience, and location.

     

    Benefits

    You will be reimbursed for the mileage you travel from the office to participant’s homes.

     

    You will receive a monthly phone stipend.

     

    On the first of the month following 60 days of employment you will be eligible for health insurance, dental insurance, vision insurance, life insurance, as well as Flexible Spending Account (FSA).

     

    Leave accrues starting on your first day of employment, and you can take paid vacation, sick and holiday leave after 90 days.

     

    We offer a 401(k) matching up to 4% after 1 year of employment and 1,000 hours worked.
