Home & Health Care Management is the oldest home health care agency in Northern California. Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients.
The Role of the Social Worker
This position is responsible for overseeing the comprehensive care management activities of clients for contracts that the agency has with the Medi-Cal Home and Community Based Alternatives (HCBA) Waiver Program and AIDS Waiver Program (MCWP). Our agency manages providers and clients in an 8-county catchment area for the California Department of Health Care Services. Our program is growing and we need additional staff to assist the agency in growing and promoting quality patient care.
Who You Are:
You have a Bachelor’s degree in Social Work or an equivalent and are passionate about making a difference in the lives of clients with chronic health conditions such as HIV/Diabetes/Heart Failure.
You are detail oriented, computer literate, and want to work as part of a management team supervising the care of clients who are receiving home and community-based services.
You are knowledgeable about community resources and helping clients solve social determinants of health care problems and want to be valued for your social workers skills.

Service Region/ Location:
The role serves clients in Butte, Glen, Yuba, Sutter, Colusa, Tehama, Shasta and Trinity counties.
This is not a remote role and requires you to work in our Chico office.
Schedule:
This role is full-time, 40 hours per week, from 8am to 5pm Monday through Friday with an hour for lunch from 12pm-1pm. No weekend work is required.
Travel:
Home health care requires you to drive to patient’s homes. You will need to be in the field approximately 50% of your time, and in the office for the other 50% of your time, depending on caseload needs.
Physical Requirements:
Conditional upon job offer is successfully passing a medical exam that tests amongst other things the ability to move, visual acuity, and hearing acuity. Physical requirement for this role specifically include
Compensation:
Pay range is $22 to $30 per hour. Exact compensation may vary based on skills, experience, and location.
Benefits
You will be reimbursed for the mileage you travel from the office to participant’s homes.
You will receive a monthly phone stipend.
On the first of the month following 60 days of employment you will be eligible for health insurance, dental insurance, vision insurance, life insurance, as well as Flexible Spending Account (FSA).
Leave accrues starting on your first day of employment, and you can take paid vacation, sick and holiday leave after 90 days.
