RN Case Manager

We are actively searching for a committed Registered Nurse to fill a PRN position in our Home Health team. The role involves a range of duties including start of care evaluations, wound care, managing medications, patient education, care of foley catheters, and ostomy management. Compensation is provided on a per-visit basis. There is a possibility for the position to evolve into a full-time, salaried role should the candidate be interested in full-time employment.

100% - Field Tasks (Caring for clients in their home)

What you'll do

• Complete patient care management
• Represent the agency to our patients and their loved ones as a genuine care partner.
• Admission, case management, supervisory/leadership to other agency staff on the client’s care team.
• Confer with physician in developing/adjusting plan of treatment
• Update and implement changes to care plan accordingly
• Provide hands-on care, and in-person training, and evaluation of the care plan
• Coordinate with office team to ensure appropriate care between various healthcare personnel (such as PT, OT, HHAs and external providers)
• Report patient care/condition/progress to patient's physician and agency leadership on a continuous basis
• Coordinate/supervise the work of your client’s care team when applicable. (CNAs)
• Monitor the appropriate completion of documentation by the care team
• Prepare medical documentation, including any case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes
• Prepare visit/shift reports, maintains patient records, and confers with other health care disciplines in providing optimum patient care

Pre-Employment Requirements

• Be at least 21 years old
• Diploma, Associate, or Bachelor Degree in Nursing
• Current and unrestricted Registered Nurse licensure
• 2 Part TB test
• Wellness Check / Physical
• Current CPR/AED
• Background check - fingerprinting (obtained upon hire)
• Strong organizational and communication skills
• A valid driver’s license, auto insurance, and reliable transportation are required

Preferred Qualifications

• 1-3 years of relevant experience
• Bilingual a plus

Job Type: Full-time

Pay: $40.00 - $50.00 per hour

Expected hours: 30 – 50 per week
