Job Overview:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for establishing relationship with referral partners and other case managers. Your role will involve coordinating care, advocating for patients, and ensuring that their needs are met.
Duties:
- Conduct comprehensive assessments of patients' needs and develop individualized care plans
- Collaborate with healthcare professionals to coordinate patient care and ensure continuity of services
- Provide counseling and emotional support to patients and their families
- Assist patients in accessing community resources and support services
- Monitor patient progress and make necessary adjustments to care plans as needed
- Document all interactions, interventions, and outcomes in accordance with HIPAA regulations
- Participate in multidisciplinary team meetings to discuss patient cases and develop treatment plans
- Build relationships with referral sources
- Visiting Assisted Living Facilities, Hospitals, Nursing homes , Rehabilitation center, Doctors offices to speak about our services and establish a referral relationship

This position is per diem starting out with about 10 hours per week and will in hours as the company grows.
