Transplant Nurse Coordinator jobs in Maine

Transplant Nurse Coordinator provides planning and coordination of all facets of clinical care for potential and actual transplant candidates and recipients. Manages referral, evaluation, and pre-transplant data for transplant patients and coordinates the clinical management of post-transplant patients. Being a Transplant Nurse Coordinator develops materials and delivers programs to educate and support patients, families, and clinicians throughout the transplant process. Requires clinical experience in the care and treatment of transplant patients. Additionally, Transplant Nurse Coordinator requires a bachelor's degree of Nursing. Typically reports to a manager or head of a unit/department. Requires registered nurse (RN). May require certification from a professional organ transplant nursing certification board. The Transplant Nurse Coordinator gains exposure to some of the complex tasks within the job function. Occasionally directed in several aspects of the work.     • Position Summary
      • The Post-Transplant Coordinator (PoTC) is responsible for coordinating patient care after transplant. Works in collaboration with the transplant nephrologists and transplant surgeon. Participates as a member of the multi disciplinary solid organ transplant team. Assumes responsibility to ensure the smooth and efficient delivery of health care services from the point of transplantation and throughout the transplant maintenance phase. Collaborates with the team to organize health care needs/services analyze and records data accurately in the health care records. Maintains compliance with applicable state, federal and UNOS regulations including the submission of patient and transplant update forms.
    • Required Minimum Knowledge, Skills, and Abilities (KSAs)
      • Education: Bachelor's degree preferred.
      • License/Certifications: Maintain licensure and certification of professional stature.
      • Experience: RN, PA, NP, recent critical care or other related experience.
      • Strong verbal, written and interpersonal skills.
      • Ability to work independently, in group situations and with the public.
      • Ability to organize and prioritize work to meet deadlines.
      • Ability to work under stress with numerous interruptions, distractions and changing priorities.
