    Woodward Resource Center, a State of Iowa Health & Human Services (HHS) facility, is seeking a Social Worker 3 to join our team. This position works remotely with occasional overnight travel.

    What You'll Do

    This position will work to improve transition outcomes, enhance services provided in the community to individuals living with intellectual disability, expand comprehensive statewide access to support services, adapt approaches to health risk management and excellence in community, and increase awareness about, public confidence in, and use of coordinated care for individuals with intellectual disability.

    WRC's Center of Excellence team will provide services that promote community integration for individuals with intellectual disabilities by providing information, coaching, and support services that assist community providers in improving their excellence in services for better outcomes, above the regulatory requirements for medical billing.

    This position will promote improved community service outcomes, including reducing risk for individuals with an intellectual disability, by helping community providers identify barriers to healthy living in person-centered planning. The Social Worker 3 will improve systematic support to community providers to serve individuals in integrated settings with complex care needs by offering consulting services, assessment, training, and assistance with plan development.

    The Social Worker 3 will support community providers who have accepted Glenwood Resource Center (GRC) Transitions during closure, providing personalized, formalized, and comprehensive support for individuals through a full 365-day plan of transition monitoring outlined at the time of transition from GRC and included in the DOJ/HHS/GRC Consent Decree.

    This position will connect community providers and staff to education and support services so they are better equipped to identify risk(s) and initiate competent and coordinated care plans for their clients and individuals living with intellectual disabilities in the community, improving the health outcomes and ability for individuals with intellectual disability to be able to live in the most integrated community of their choice. This position will also enhance services provided in the community to individuals living with intellectual disabilities by advocating for practice change and quality oversight.

    Provide Support To Community Providers Through The Following

    • Develop and provide generalized training to community providers, as needed, including training to be made virtually.
    • Provide knowledge and recommendations within their clinical expertise, as needed.
    • Ensure quality trainings in each of their respective areas are written out with sufficiently individualized stepped out tasks.
    • Provide training in-person/virtually, when requested, to community provider staff ensuring a competency-based approach with return demonstration/practice when possible, and if not appropriate to the subject/task, then a written test of competency.
    • Ensure quality training is provided to community provider staff through improved step by step task list trainings, written testing of understanding, and competency-based training through return demonstration of skills prior to transition of individuals to the community provider's home.
    • Provide quality training with train the trainer approach for supervisory staff or clinical staff from the community.
    • Complete all training with signed training sheets including appropriate supplemental documentation attached.

    Must possess and maintain a valid Driver License and be able to drive a state vehicle.

    Minimum Qualification Requirements

    Graduation from an accredited college or university with a Bachelor's degree and the equivalent of three years of full-time experience in a social work capacity in a public or private agency;

    OR

    graduation from an accredited college or university with a Bachelor's degree in social work and the equivalent of two years of full-time experience in a social work capacity in a public or private agency;

    OR

    a Master's degree in social work from an accredited college or university;

    OR

    an equivalent combination of graduate education in the social or behavioral sciences from an accredited college or university and qualifying experience up to a maximum of thirty semester hours for one year of the required experience;

    OR

    employees with current continuous experience in the state executive branch that includes the equivalent of one year of full-time experience as a Social Worker 2 shall be considered as qualified.

    Additional Qualification Requirements

    For designated positions in case management, the appointing authority may request those applicants possessing a Bachelor's degree from an accredited college or university with a major or at least 30 semester hours or its equivalent in the behavioral sciences, education, health care, human services administration, or social sciences and the equivalent of 12 months of full-time experience in the delivery of human services in the combination of: chronic mental illness, developmental disabilities, and intellectual disabilities as a Targeted (Medicaid) Case Manager;

    OR

    an Iowa license to practice as a registered nurse and the equivalent of three years of full-time nursing or human services experience with the above population groups.
