Job Summary:
The Home Care Nurse, under the direction of a physician or nurse manager, provides skilled nursing care to patients in their homes. The role involves assessing patient needs, developing care plans, administering medications, providing wound care, monitoring vital signs, and educating patients and their families about health management.

Key Responsibilities:

• Conduct comprehensive assessments of patients' health status and needs.
• Develop individualized care plans in collaboration with the patient, their family, and healthcare team members.
• Perform skilled nursing procedures such as wound care, medication administration, and intravenous therapy.
• Monitor patients' vital signs, symptoms, and overall condition during home visits.
• Coordinate with other healthcare professionals, including physicians, therapists, and social workers, to ensure holistic patient care.
• Educate patients and their families about disease management, treatment options, medication usage, and lifestyle modifications.
• Provide emotional support and counseling to patients and their families, addressing their concerns and promoting coping strategies.
• Document all care provided accurately and timely in electronic medical records (EMR) systems.
• Evaluate the effectiveness of interventions and adjust care plans as needed to achieve optimal patient outcomes.
• Ensure compliance with regulatory standards, policies, and procedures related to home healthcare services.
• Participate in interdisciplinary team meetings and continuing education to enhance clinical skills and knowledge.

Qualifications:

• Valid Registered Nurse (RN) license in the state of practice.
• Bachelor of Science in Nursing (BSN) preferred.
• Previous experience in home health, community health, or a related clinical setting preferred.
• Strong assessment, critical thinking, and communication skills.
• Ability to work autonomously and make independent clinical judgments.
• Compassionate and patient-centered approach to care.
• Proficiency in electronic medical record documentation.
• Reliable transportation and a valid driver's license.
• CPR certification.
• Ability to lift and move patients as needed.

Working Conditions:
Home care nurses typically work variable hours, including evenings, weekends, and holidays, to accommodate patient needs. They may travel extensively within their designated service area to conduct home visits. The job can involve exposure to infectious diseases and physical demands such as bending, lifting, and standing for extended periods.
