Social Services Director - Nursing Home in a nursing home environment, directs, establishes, and plans the overall policies and goals for the social services department. Job Description: We are currently seeking a dedicated and compassionate Social Services Assistant to join our team at Pearl City Nursing Home. The Social Services Assistant will play a crucial role in providing essential support to our residents and their families, ensuring they receive the highest level of care and assistance during their stay with us.
Responsibilities:
- Assist the Social Services Director in coordinating and implementing social service programs.
- Provide emotional support and counseling to residents and their families, addressing their social and emotional needs.
- Collaborate with interdisciplinary teams to ensure comprehensive care coordination for residents.
- Assist residents with accessing community resources and services to enhance their quality of life.
- Participate in care conferences and meetings to discuss resident care plans and updates.
- Advocate for residents' rights and ensure their voices are heard.
Qualifications:
- Previous experience working in a healthcare or long-term care setting is desirable.
- Strong interpersonal and communication skills, with the ability to interact compassionately with residents and their families.
- Empathetic and patient-centered approach to care.
- Ability to work effectively as part of a multidisciplinary team.
- Excellent organizational skills and attention to detail.
- Knowledge of community resources and social service agencies preferred.
- Ability to maintain confidentiality and professionalism at all times.
- Social Services designee certificatin preferred.
- Associate degree in human services preferred.
