Pharmacist - Home Care compounds and dispenses prescribed medications and pharmaceutical supplies per physician's instructions for patients in a home care setting. Provides consultation and answers inquiries from patients, families, physicians and other healthcare professionals regarding drugs and their use. Being a Pharmacist - Home Care maintains all required documentation and data on prescriptions, inventory, and patients. Adheres to the Joint Commission standards for Home Care and complies with all regulatory guidelines and procedures. Additionally, Pharmacist - Home Care requires a doctorate degree in pharmacy. Requires Registered Pharmacist (RPh). Typically reports to a manager. Pharmacist - Home Care's years of experience requirement may be unspecified.     • New hires are placed on our starting wage scale of $119,995 to $150,000 per year.
    • Based on performance, there is room to grow on our full wage scale of $119,995 to $180,000 per year.

    Work Schedule:

    • 5 8-hour shifts Monday-Friday with a Saturday rotation

    Job Summary


    The Pharmacist is responsible for dispensing of prescriptions and inventory control, medication therapy management, and providing support and consultation services to medical, behavioral health, and dental providers, and health center support staff.

    Requirements

    • Possesses advanced general skills, including written and verbal communications skills, computational and computer skills, and mathematical knowledge at a level typically acquired through completion of a general bachelor’s degree program.
    • Possesses the equivalent of the highest level of extensive formal training in Pharmacy including an understanding of the application of the theory and practices of the profession to the operations of the organization, usually in the form of a doctoral or post-doctoral degree (or other applicable extended training program).
    • Possesses a full understanding of the theory behind how procedures and practices in Pharmacy are developed, beyond how they are used at UCNW, in order to effectively make significant contributions to policy and procedure development.
    • Experience with disease state management, precepting pharmacy students, and collaborative work with primary care providers and nursing staff desired. 
    • Washington State Pharmacist license required.
    • Current Basic Life Support (BLS) certification required - this training can be offerred free of charge upon hire.
    • Knowledge of State and Federal regulations.
    • Knowledge of Pharmacy Quality Assurance Commission rules.
    • Knowledge of 340B program.
    • Ability to work with primary care providers to maximize cost effective prescribing patterns, improve clinical processes, and achieve clinical standards across the population of patients served by UCNW.
    • Proficiency in Microsoft Office programs including Word, Outlook and Excel.
    • Keyboarding speed of 30 wpm and data entry skills; accuracy is essential.
