Job Summary :
Administers, directs, and coordinates activities of Patient Care and is responsible for the recommendation, development and implementation of policies and procedures in these areas, and for keeping the ACNO informed of both progress and potential problems.
Responsible for the direction and evaluation of patient care delivery.
The MDS Assessment Coordinator is responsible and accountable for completion and coordination of the MDS and interdisciplinary care plan on all patients admitted to the Transitional Care Unit.
Qualifications :
Graduation from an accredited Nursing School as an LPN or RN
5 years experience in Long Term Care / SNF to include 2 years MDS experience (preferred). LPN required, RN preferred
NM LPN or RN License or compact state license.
BLS Certification or obtain per policy upon hire; maintain such certifications.
Associate's degree required.
1-3 years experience in related field.
Health care experience required.
LPN or RN License required.
