Minimum Data Set (MDS) Coordinator (LPN) assesses long-term care patient health needs in collaboration with the Minimum Data Set (MDS) Coordinator (RN). Assists in developing a comprehensive care plan. Being a Minimum Data Set (MDS) Coordinator (LPN) maintains medical records and ensures documentation is present for coding. Adheres to federal, state, and local regulations and organizational policies and procedures. Additionally, Minimum Data Set (MDS) Coordinator (LPN) requires a high school diploma. Requires Cardiopulmonary Resuscitation (CPR) and Licensed Practical Nurse (LPN) certification. Typically reports to a mds rn. Minimum Data Set (MDS) Coordinator (LPN)'s years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. MDS Coordinator's responsibilities include:- Provide accurate documentation of all services to assure accurate reimbursement from all payor sources.
- Ensure that needed resources are available and that quality care is delivered to all residents
- Coordinate the development of a written care plan and assessment for each resident and review and revise as appropriate.
- Must have a current RN or LPN license
- Must have Long Term Care experience
- 1 - 2 years MDS experience preferred
- Must be knowledgeable of long-term care nursing and medical practices and procedures
