Reports to : Medical Records Clerk Supervisor
POSITION SUMMARY:
Responds to internal and external requests for medical records adhering to all medical records policies and guidelines. Processes medical records following patient visits.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Scan all appropriate forms to the electronic medical records.
- Answers and responds to medical record inquiries, prioritizing problems, screening calls, taking messages and relaying information. Within the scope of authority, provides information to callers.
- Responds to requests for medical records, adhering to legal requirements and patient confidentiality.
- Implements medical records policies and procedures.
- Checks voicemail for patient release of information. Complete training on how to use the copy, fax, and scanning machine.
- Assigns electronic labs accurately by identifying patient name/ date of birth.
- Electronic faxing.
- Incoming/Outgoing Mail Process
- Separate incoming mail by department (hand deliver to the POD’s)
- Collect all outgoing mail by department (forward to mail room to be stamped)
- other related duties as assigned.
MINIMUM SKILLS AND KNOWLEDGE REQUIREMENTS:
High School diploma or equivalent required. Ability to relate to patients and other staff in a professional manner. Good organizational skills and the ability to prioritize workload. Good telephone manner and the ability to alphabetize accurately. Familiarity with the computer. Bilingual capability desired.
WORKING CONDITIONS:
Works in a busy community health center in an urban neighborhood. Occasional long hours may be required.
PHYSICAL REQUIREMENTS:
Physical demands require lifting small pieces of equipment and/or boxes not over 20 lbs. Mostly standing, sitting, and interoffice walking required. Visual acuity sufficient for frequent reading. Hearing acuity sufficient for holding conversations with or without audio devices.
