Role Overview
As a Nurse Care Manager at Compass Health Network, you will be an integral part of our integrated care team, focusing on delivering individual and population-based health interventions to individuals enrolled in our Health Care Home program. By blending physical health and behavioral health care, you will help improve the overall well-being of our clients.
Responsibilities- Manage a case load of up to 200 Health Care Home clients, ensuring completion of monthly interventions and adherence to DMH regulations.
- Conduct health screenings, identify risks, set care goals, and promote healthy lifestyles.
- Facilitate client referrals to primary care physicians or specialists.
- Perform metabolic screenings and medication reconciliations for clients.
- Educate clients and caregivers on health conditions, treatment, and prevention.
- Collaborate with medical providers, community-based agencies, and other support services to deliver holistic care.
- Attend team meetings, training sessions, and agency events as required.
- Travel to clinics, client homes, and other locations for nursing coverage.
- High School/GED required.
- LPN or RN licensure.
- Practical Nursing certificate or nursing degree.
- Proficiency in Microsoft Office Suite and Electronic Medical Records preferred.
