Job Overview:
We are currently looking for a motivated and compassionate individual to fill the role of Home Care Agency Intake Coordinator and Onboarding Specialist. In this hybrid role, you will be responsible for managing the intake process for new clients and coordinating their onboarding and orientation to our home care services.
Duties:
Intake Coordinator:
- Receive and review referrals for home care services and conduct initial assessments of potential clients
- Gather necessary information from clients, families, and healthcare professionals to determine care needs and eligibility
- Coordinate with internal team members to develop personalized care plans and schedules for new clients
- Maintain accurate and up-to-date client records and admissions status
- Communicate with clients and families to provide guidance and support throughout the intake process
- Collaborate with billing and insurance departments to verify coverage and obtain authorizations
- Ensure a smooth transition for clients into our care services
Onboarding Specialist:
- Create and implement a comprehensive onboarding process for new clients to familiarize them with our services and care team
- Coordinate with field staff to schedule caregiver assignments and provide necessary training and orientation
- Conduct follow-up assessments with clients and families to address any issues or concerns and ensure satisfaction
- Educate clients and families on our care protocols, policies, and resources available to support their well-being
- Monitor and evaluate client progress and adjust care plans as needed
- Provide ongoing support and communication to clients and families to foster positive relationships and quality care experiences
Qualifications:
- Bachelor's degree in social work, nursing, healthcare administration, or related field (preferred)
- Previous experience in intake coordination, onboarding, or customer service within a healthcare setting
- Strong interpersonal and communication skills to build rapport with clients and families
- Detail-oriented with excellent organizational and time management abilities
- Knowledge of home care services and regulations is advantageous
- Proficiency in computer applications and electronic health records systems
- Ability to work effectively both independently and collaboratively in a fast-paced environment
