Primary Function/General Purpose of Position
This position provides operational leadership to manage Population Health Clinical Services programs. The program manager will offer support to Clinical Services leaders to enhance and optimize operations, implement new initiatives and manage performance data for care coordination, advance care planning, and post-acute care services teams.
Essential Job Functions
In collaboration with Clinical Services leaders, work with local market leaders and care coordination staff to implement new programs or services
In collaboration with leaders, create and record standardized processes and policies and update annually or as needed
Supports Clinical Services leaders with onboarding of new staff
Works with leaders and analytics to process and share performance data and support the development of tools for the alignment of measures and timely reporting
Under the direction of clinical services leadership, support the development of strategic initiatives to improve results of value-based contracts.
Actively participate in Board, its committees and management committees and workgroups as requested
Perform necessary analysis, investigation and evaluation to recommend and support project activities and tasks as defined by project work plans and schedules to support system-wide best practices
Develop project charters and workplans and engage stakeholders for new partnerships, collaborative activities and other implementations to support KPIs and ministry objectives

Education

Work Experience
1 year of experience managing projects or supporting initiative implementation (required)
2 years of recent experience in healthcare setting (preferred)
1 year of population health or care coordination experience (preferred)
