Reviews the patient's case and determines the best choice in services and facility to meet the medical and social needs of the patient. Being a Discharge Planner - Home Care ensures a safe transfer to a setting that meets the patient's needs and coordinates necessary services and staff required to complete the transfer. May require a bachelor's degree. Additionally, Discharge Planner - Home Care typically reports to a supervisor or manager. The Discharge Planner - Home Care gains exposure to some of the complex tasks within the job function. Occasionally directed in several aspects of the work.     Location: Chantilly Virginia 

    Work Environment: Medical Detox/ Residential Treatment Center 

    Job Type: Full Time 

    Schedule: Tuesday -Saturday 8:30am-5:00pm

    License Requirements: LBSW OR similar license/Education Status is strongly preferred. 

    Compensation: $18-24 Hourly

    -Work closely with clients and the care team to assess their progress and needs
    -Develop a customized plan that ensures a smooth transition to the next level of care

    - Act as an influencer for clients to motivate them to follow their plan and achieve their goals
    - Provide support, guidance, and education to clients and their families
    - Create discharge summaries, document progress, and communicate with referral sources

    Qualifications: 

    • 2 years of experience with treatment of substance abuse and/or co-occurring disorders is required.
    • 2 years of experience with Discharge Planning and/or Case Management in required.
    • Bachelor's degree in psychology or related field is strongly preferred.
    • Previous experience in Salesforce and/or EHR systems is strongly preferred.
    • License Requirements: LBSW OR similar license/Education Status is strongly preferred. 
    • Candidates Must Be:

      • Able to manage their time well.
      • Able to function independently without direct supervision.
