Discharge Planner - Home Care coordinates the movement of patients between a hospital or skilled nursing facility and a home care or hospice arrangement. Reviews the patient's case and determines the best choice in services and facility to meet the medical and social needs of the patient. Being a Discharge Planner - Home Care ensures a safe transfer to a setting that meets the patient's needs and coordinates necessary services and staff required to complete the transfer. May require a bachelor's degree. Additionally, Discharge Planner - Home Care typically reports to a supervisor or manager. The Discharge Planner - Home Care gains exposure to some of the complex tasks within the job function. Occasionally directed in several aspects of the work. Provides psychosocial assessments, crisis intervention, resources referrals, to facilitate discharge plans, and/or adjustment to illness, and complex discharge planning for patients and their families.
Formulates the discharge plan with patient, families and the care team based upon a needs assessment.
Coordinates appropriate referrals to home care agencies, skilled nursing and rehabilitation centers, and community-based programs. Coordinates care authorization process with insurers.
Coordinates specific details of patient's hospitalization with Utilization Management department to ensure appropriate admission status.
3 - 5 years health care experience preferred
Requires a Master Degree in Social Work (MSW)/ or BSW or a licensed RN, BSN preferred
Current NJ SW License, LCSW Preferred.

Current NJ RN License
ACM (American Case Management Association) preffered
CCMC (Commission for Case Manager Certification) preffered
