Reviews the patient's case and determines the best choice in services and facility to meet the medical and social needs of the patient. Being a Discharge Planner - Home Care ensures a safe transfer to a setting that meets the patient's needs and coordinates necessary services and staff required to complete the transfer. May require a bachelor's degree. Additionally, Discharge Planner - Home Care typically reports to a supervisor or manager. Shift Time: AM (flexible), Monday-Friday
Starting at $20-40/hour (wage increase pending experience); Health, Vision, Dental benefits, 401K, etc available.
Discharge Planner Job Duties: work with family and patients to develop safe discharge especially for our long term patients, keep updated logs, document daily, complete assessments, follow Title 22 guidelines and regulations, etc.
