Provides information and referrals to patients/families regarding resources and services, i.e., financial, equipment, home health, nursing home, etc. Participates in formal and informal interdisciplinary staffing regarding patient care and care planning. Documents assessment, interventions and plan according to JCAHO standards. Identifies discharge planning/teaching needs upon admission. Assists with developing and coordinating discharge plans and arranges post-discharge services as appropriate for patient needs. Monitors and evaluates case management program effectiveness. Performs other duties as assigned or needed to meet the needs of the department/organization.
