Field Care Coordinator (Home Care/Bilingual)

  • Summary

    Daily and weekly execution of active Emblem Home Care client home visits. Directly support Service Compliance Officers byfacilitating monthly and annual home visits as outlined by The State of Arizona ALTCS AHCCCS. This position requires that you drive throughout Maricopa County and be willing to expand into other counties within Arizona. Will require local travel by car back and forth between client homes and the office on a weekly basis. Attend staff meetings multiple times per week and in office coordination while in the field working with clients. Must have a firm working knowledge of home and community-based support services within Arizona. Must be able to maintain a clean driving record and carry car insurance minimums as dictated by department of motor vehicles in Arizona. Will be responsible for the execution of documentation in the home or other location (s) for all active clients within Emblem Home Care.  

    Essential Operations Duties and Responsibilities


    1. Maintain working knowledge of Arizona Long Term Care regulations as they pertain to Home and Community Based Services. Staying up to speed on any changes to Arizona Case Management Handbook and AHCCCS Medical Policy Manual (AMPM)
    2. Advocate on the Clients behalf with all involved agencies, MCO, organizations, and individuals. 
    3. Reassesses the Clients and/or Caregiver’s service need every 30, 60, 90, and 120 days or as needed. 
    4. Responsible for the documentation and communication to Services Compliance Officers, Operations Team Members, and MCO Case Manager of any incidents that occurred while at home visit or have been communicated to Field Compliance Officer during the time of the home visit. Documentation includes but not limited to, care plan, home monitoring form, emergency questionnaire, daily activity notes, medication list, and skin breakdown form.
    5. Responsible for ensuring that Clients and Caregiver or informal support system receives appropriate services and that the services received are of high quality.
    6. Maintain quality documentation in paper, electronic, and or on all devices used during client home visit.
    7. Responsible for documenting updates to clients Service Compliance Officer after each home visit and following up to ensure questions or concerns have been rectified on client’s behalf.
    8. Partner with Operations Staff to determine staffing needs.
    9. Serves as an expert for recruiting candidates for the agency.
    10. Uses traditional and non-traditional resources to identify and attract quality candidates such as career fairs, on-line job fairs, community network events, etc.
    11. Perform cold calls to attract potential candidates.
    12. Ensure screening, hiring, and selection of candidates are completed in accordance with 1099 Idependant Contractor laws and regulations as they pertain to Arizona and ALTCS rules and regulations.
    13. Follows up with candidates to ensure updated information on the onboard status.
    14. Manages onboarding by scheduling new contractors to onboard in the office and assist with completion of electronic onboard through WorkMarket software.
    15. Assists contractors with access to agency software applications used for payment, scheduling, and training.
    16. Communicates important contractor information during delivery of contract offers (i.e. 1099 Contractor Model, communication expectations, and continuation of contract information, etc.)

     Qualifications and Skills


    1. High School Diploma or GED
    2. Bi-Lingual Preferred
    3. Experience in dealing with third party-provided services
    4. Operational ability in a diverse, large-scale environment
    5. Exceptional customer-facing skills
    6. Strong organizational skills
    7. Capacity to train and guide junior team members
    8. Ability to manage and prioritize tasks efficiently
    9. Solid resource planning and problem-solving aptitudes
    10. Readiness to demonstrate a proactive attitude
    11. Excellent verbal and written communication skills
    12. Must be able to obtain a Level One Fingerprint Clearance Card
    13. Must have a good standing driving record and driver license
    14. Must be able to obtain all credentials set-forth by Arizona AHCCCS
    15. Must have excellent communication skills and ability to work independently and both as a team member.
