Case Manager jobs in Wyoming

We are seeking a dedicated and organized Case Manager to join our counseling team. The Case manager will be responsible for coordinating client services, managing schedules, ensuring effective communication, and facilitating transportation. This role requires exceptional communication skills, proficiency with various computer programs, and strong organizational abilities to support our clients and counseling staff effectively.

Key Responsibilities:

-Serve as the primary point of contact for clients

-Maintain comprehensive case plans developed by clinicians for each assigned client

-Monitor and document client progress, working with clinicians to adjust care plans accordingly

-Coordinate and schedule client appointments

-Send appointment reminders and follow up with clients regarding missed appointments

-Arrange and coordinate transportation for clients as needed

-Track and manage transportation schedules

-Maintain communication between clients, families and service providers

-Prepare and distribute written communications including reports and updates

-Handle sensitive information with confidentiality and professionalism

-Maintain accurate and up-to-date client records in compliance with legal standards

-Ensure accurate data entry for client records

-Collaborate with clinicians to ensure comprehensive delivery of services

-Participate in team meetings and contribute to case discussions

Qualifications: 

-A high school diploma or GED

-Excellent written and verbal communication skills

-Strong organizational and time management abilities

-Ability to multi-task

-Compassionate and client-focused, with the ability to build rapport and maintain professional relationships

-problem-solving skills and the ability to work independently and as part of a team

Other Requirements:

-A valid driver license, reliable transportation and clean driving record

-Ability to pass a background screening

-Flexibility to work evenings and weekends is a plus
