Case Manager coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. Monitors care and acts as a liaison between patient/family, healthcare personnel, and insurers. Being a Case Manager evaluates the needs of the patient, the resources available, and recommends and facilitates the plan for the best outcome. Develops a discharge plan that provides the best available resources to meet ongoing patient needs and that encourages compliance with medical advice. Additionally, Case Manager identifies patient care issues and suggests revisions to or new clinical pathways to improve quality of care. May also be responsible for conducting utilization reviews. Typically requires a bachelor's degree of Nursing. Typically reports to a head of a department/unit. Requires Registered Nurse (RN). Case Manager's years of experience requirement may be unspecified. Established in 2013, Brighter Transition YTC is a step down, transition program for males age 16-20 that have completed a long-term residential program. Our campus is located 7 miles west of Sioux Falls, South Dakota. Our academy offers a year-round academic program provided by the local school district. Currently, Brighter Transition YTC serves state of South Dakota Department of Corrections commitments.
The Job
The primary function of the Case Manager is to write and prepare all initial care plans, updated service plans, progress reports (as needed) and discharge summaries in a timely manner in accordance with State of South Dakota Regulations. Maintain safety, accountability (correct count of each student) and security of campus. Coordinate with entire team regarding admission, discharge and transportation of youth. Act as a resource person from team to outside systems including referring workers, families, schools and other stakeholders. Coordinate all activities with court and referring agencies regarding admissions and discharges. Maintain an ongoing awareness of students’ behavior and progress in the program. Participate as an active team member.
