Case Manager - Home Care/Hospice coordinates the overall interdisciplinary care plans for home health/hospice patients from admission to discharge. Acts as the liaison between patient/family and homecare personnel to ensure necessary care is provided promptly and effectively. Being a Case Manager - Home Care/Hospice requires a bachelor's degree. Typically reports to a manager. Additionally, Case Manager - Home Care/Hospice requires RN license. Case Manager - Home Care/Hospice's years of experience requirement may be unspecified. Summary of Work
The primary responsibilities of this position are to provide In Home and Foster Care case management to children and families in Buffalo Bridges Human Service Zone (Barnes & Stutsman Counties). You may also be required to provide these services in other zones. This will include:
Minimum Qualifications
Requires a bachelor's degree in social work, psychology, sociology, counseling, human development, elementary education, early childhood education, special education, child development and family science, human resource management (human service track), or criminal justice, and two years of professional human services work experiences as a social worker, child protective service worker, or professional case manager. A Master's degree in one of the areas above may substitute for one year of the work experience requirement.
If no qualified applicants are found, we will consider under-filling this position as a Family Service Specialist I - with a minimum salary of $3508.00 per month.
Minimum qualifications require a bachelor's degree in social work, psychology, sociology, counseling, human development, elementary education, early childhood education, special education, child development and family science, human resource management (human service track), or criminal justice, and one year of professional human services work experience as a social worker, child protective service worker, or professional case manager; or a Master's degree in one of the areas above.
Individuals may be required to work non-traditional work hours. The ability to visit persons in their own homes and providing transportation for clients using the employee's own transportation is
required for this position. A criminal background check and a driver's record check will be performed prior to employment.
