Case Manager - Home Care/Hospice coordinates the overall interdisciplinary care plans for home health/hospice patients from admission to discharge. Acts as the liaison between patient/family and homecare personnel to ensure necessary care is provided promptly and effectively. Being a Case Manager - Home Care/Hospice requires a bachelor's degree. Typically reports to a manager. Additionally, Case Manager - Home Care/Hospice requires RN license. Case Manager - Home Care/Hospice's years of experience requirement may be unspecified. Phoenix Home Care & Hospice is seeking a qualified Registered Nurse in the Lake Ozark and surrounding areas!
As a Home Health RN Case Manager, you will be the first point of contact between a patient and hospital. The admission RN should possess strong skills in physical assessment, diagnostic interpretation, and communication to formulate an individualized plan of care according to physician orders that incorporates the analysis of initial assessment. Working alongside other nurses, Physical Therapist, Speech Therapist and or Occupational Therapist.
Pay Range: $65,000 - $90,000 based on 30 units.
Shift Availability: Full Time, Monday - Friday: 8am - 5pm. This position carries On Call responsibilities every 8 - 12 weeks.

- Communication with physicians to obtain orders and write diagnosis driven plans of care
- Work alongside strong therapy team including PT/OT/Speech
- Active RN Nursing and Driver's License
