The Support Coordinator will conduct initial CPOC meetings and complete quarterly review meetings. The Support Coordinator is responsible for completing monthly phone contacts with families and providing resource advocacy to connect the family to local and state resources. Prepare comprehensive plans of care for children and adults in the Region 6 area (Avoyelles, Catahoula, Concordia, Grant, LaSalle, Rapides, Vernon and Winn). Travel required throughout Region VI. At least Bachelor's degree in Human Services Field required. Previous experience as Support Coordinator preferred.
