Case Manager - Home Care/Hospice coordinates the overall interdisciplinary care plans for home health/hospice patients from admission to discharge. Responsibilities:
- Conduct comprehensive assessments of patients' medical, social, and psychological needs
- Develop individualized care plans in collaboration with the interdisciplinary team
- Coordinate and facilitate communication between patients, families, and healthcare providers
- Provide counseling and support to patients and their families
- Monitor patient progress and adjust care plans as necessary
- Collaborate with community resources to ensure continuity of care
- Advocate for patients' rights and ensure they receive appropriate services
- Document all interactions and interventions accurately and timely
Requirements:
- Bachelor's degree in social work or related field
- Experience in case management or related field (6 months paid experience required)
- Knowledge of medical terminology and healthcare systems
- Familiarity with IDD provider settings
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
- Proficiency in managed care principles and practices
- Understanding of mental health conditions and referral techniques
- Knowledge of discharge planning processes
- Ability to handle stressful situations with empathy and professionalism
