We are seeking a compassionate and skilled Home Health Registered Nurse (RN) to join our team in The Villages, FL. The Home Health RN will be responsible for providing comprehensive nursing care to patients in their homes, assessing their needs, developing care plans, and coordinating with other healthcare professionals to ensure the delivery of high-quality, patient-centered care. **Responsibilities:**
1. Conduct comprehensive assessments of patients' health status, including physical, emotional, and psychosocial factors.
2. Develop individualized care plans in collaboration with patients, families, and interdisciplinary team members, based on assessment findings and patient goals.
3. Provide skilled nursing care, including wound care, medication management, IV therapy, and disease management, as prescribed by physicians.
4. Monitor patients' condition and response to treatment, and modify care plans as needed to achieve desired outcomes.
5. Educate patients and caregivers about their health conditions, medications, self-care techniques, and safety precautions.
6. Coordinate with other healthcare professionals, such as therapists, social workers, and home health aides, to ensure continuity of care and optimal patient outcomes.
7. Communicate effectively with patients, families, and healthcare team members to coordinate care, address concerns, and promote patient satisfaction.
8. Document all patient assessments, interventions, and outcomes accurately and timely in the electronic health record system.
9. Adhere to regulatory requirements, agency policies, and professional standards of practice in delivering home health services.
10. Participate in quality improvement initiatives, clinical meetings, and continuing education activities to enhance nursing practice and improve patient care.
**Qualifications:**
1. Registered Nurse (RN) licensure in the state of Florida required
2. Minimum of 2 years of clinical experience, preferably in home health or a related setting.
3. Strong assessment and critical thinking skills, with the ability to make sound clinical judgments and prioritize care.
4. Excellent communication and interpersonal skills, with the ability to establish rapport with patients and families.
5. Proficiency in electronic health record systems and Microsoft Office Suite.
6. Ability to work independently, manage time effectively, and adapt to changing patient needs and priorities.
7. Valid driver's license and reliable transportation for home visits.
8. Commitment to providing compassionate, patient-centered care and advocating for patients' rights and dignity.
