Behavioral Health Case Management Manager jobs in Missouri

Job Purpose

The Youth Homeless Demonstration Project (YHDP) Case Manager helps youth and youth adults (17-24 years of age) find affordable, safe, quality, and accessible housing. They work with their communities to encourage the expansion of housing options.

Qualifications

• Minimum of high school diploma or equivalent.
• Demonstrates ability through prior work experience to be competent in the position.
• Lived expertise in overcoming mental health challenges/homelessness.

Core Job Duties

• Explain policies and procedures of the YHDP Subsidy Housing Program.
• Interpret qualifying information from person(s)-served and determine eligibility.
• Perform technical and administrative duties in support of Housing Programs.
• Monitor housing properties for compliance with State requirements and federal regulations.
• Recommend and assist in the implementation of goals and objectives.
• Establish schedules and methods for providing affordable housing services, while utilizing policy and procedures.
• Demonstrate a working knowledge of diagnostic criteria set for substance use and/or mental disorders.
• Adherence to all applicable evidence-based practice models.
• Orally communicate information effectively and accurately.
• Assure program operates within the constraints of the agency’s certification (CARF, DMH) standards and strategic plan related to clinical care.
• Abide by program and agency policies and procedures.
• Other job duties and special projects as assigned.
