Job Summary:
We are seeking a dedicated Care Coordinator to join our team. The Care Coordinator will be responsible for coordinating care services for individuals with developmental disabilities, dementia, and other specialized needs. Duties:
- Coordinate and oversee care plans for individuals with developmental disabilities, dementia, and other specialized needs
- Provide guidance and support to caregivers on best practices in caregiving, behavior management, and resident rights
- Collaborate with nursing home staff, home care providers, and other healthcare professionals to ensure comprehensive care
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed
- Maintain accurate documentation of care services provided and resident progress
- Implement systems to streamline care coordination processes
- Conduct assessments to identify individual needs and preferences
Skills:
- Experience in caregiving, nursing home settings, or working with people with developmental disabilities
- Knowledge of dementia care, behavior management techniques, and resident rights
- Strong organizational skills with attention to detail
- Ability to work effectively in a fast-paced environment
- Familiarity with home care services and senior care best practices
