Job Summary:
We are seeking a dedicated Care Coordinator to join our team. The Care Coordinator will be responsible for coordinating care services for individuals with developmental disabilities, dementia, and other specialized needs. This role involves working closely with residents, families, and healthcare providers to ensure the highest quality of care.

Duties:
- Coordinate and oversee care plans for individuals with developmental disabilities, dementia, and other specialized needs
- Provide guidance and support to caregivers on best practices in caregiving, behavior management, and resident rights
- Collaborate with nursing home staff, home care providers, and other healthcare professionals to ensure comprehensive care
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed
- Maintain accurate documentation of care services provided and resident progress
- Implement systems to streamline care coordination processes
- Conduct assessments to identify individual needs and preferences

Skills:
- Experience in caregiving, nursing home settings, or working with people with developmental disabilities
- Knowledge of dementia care, behavior management techniques, and resident rights
- Strong organizational skills with attention to detail
- Ability to work effectively in a fast-paced environment
- Familiarity with home care services and senior care best practices
- Excellent communication and interpersonal skills
- Ability to maintain confidentiality and professionalism at all times

Pay: $17.00 - $19.00 per hour
